Much like Titus has discovered, I'm a idiot.

Added a 'showforms' prior to filling it out.

Added 'single quotes' around the checkbox value instead of double quotes.

fv("1", "sys_req", '1')
fv("1", "terms", '1')

Added another 'showforms' after filling it out - just to make sure.

And it works.
