Yeah this happened to me to.<br>

Save a copy of form.html to your local computer (my documents, or
wherever) and access like this...<br>
<br>
<a class="moz-txt-link-freetext" href="file:///c:/documents">file:///c:/documents</a> and settings/username/my documents/form.html<br>
<br>
or something like that (I am on Linux but just google on how to access
it using <a class="moz-txt-link-freetext" href="file://">file://</a> method from local computer).<br>
