Changes:
* removed "../work" as default work directory in OS X Qt-viewer.
* apocalypse widget now displays "0.99" on open of environment control box.
* selecting 2-d topology wasn't working in simple setup assistant; fixed.

Most problematic bugs:
* build of Qt-viewer fails by creating four or five subdirectories
  named "source/<a name>.h"; these are supposed to be header files
  generated by Qt from forms.  Temporary workaround is deletion of these
  subdirectories, followed by second "make" command.
* several parts of OS X application still require hand-assembly.
* crash in core of avida when apocalypse widget kills entire population.
* renaming OS X Qt-viewer application causes crash on launch.
* Qt-viewer breaks easily given invalid path names in genesis file.
