Many changes, and probably much breakage for several cvs commits
while I work out the build bugs.

* Switched from recursive makefiles beneath subdirectory "source/"
  (back) to modular makefiles.
* Throughout Qt gui:  added standard menus to all main windows; this
  is undesirable on non-Macintosh and will shortly be removed.
* Per advice from group, updated event editor, setup assistant, 
  environment controller, map window, plot viewer, instruction viewer.
