New instruction viewer.
Features:
  scales well to large organisms, and is stable (yay!);
  buttons in `brk' field to set/clear breakpoints;
  buttons in instruction-head, read-head, write-head,
    and flow-head fields to move the heads;
  popup menu in instruction field to insert/remove/change instructions;
  color bands to indicate head positions.
Instruction viewer has been cleaned but could use more cleaning, as
evolution has left things convoluted.  Many old versions of instruction
viewers need to be removed from the project.

Tried to remove some deadlock bugs -- this may have left some data
unprotected from concurrent access.  Please stress-test and let me know
of crashes in the viewer that are difficult to trace.
