top-level `make -f Makefile.csv' triggers `bootstrap'; bootstrap will be
removed.

nested autotools projects have been removed.

changed source and library filenames to match original naming
conventions.

addition of qmakeMakefile.inc, and changes in
source/qt-viewer/Makefile.am and source/qt-viewer/exp/Makefile.am,
employ qmake in the build process.  this eases building of the Qt viewer
on new platforms.  this process required renaming header files from *.hh
to *.h, and test files from *.t.cc to *_t.cc.
